Mon Health Preston Memorial Hospital Patients Gain Access to Free FARMacy Program

Social determinants of health such as where people live, their social support, financial distress, and access to health care, education, and healthy food can impact a person’s health. Vandalia Health Network’s FARMacy Program is designed to alleviate some of these burdens, providing free resources and food for patients managing diabetes.
The FARMacy Program is offering a free, 15-week course for current patients of Mon Health Preston Memorial Hospital that will provide access to fresh produce, cooking tips, and physical education each week. By participating in the program, patients will join a community that supports the improvement of their health.

It is also important to us that our community members have access to quality foods and understand the life-changing benefits of making health-conscious choices,” said Melissa Lockwood, Chief Administrative Officer of Mon Health Preston Memorial Hospital. “We’ve seen a high demand for this program and thanks to our partnership with the Vandalia Health Network, we are able to offer this free to our patients for the second time. We look forward to expanding this opportunity to even more patients as we work to assist them in creating healthier lives.”
Those interested in participating in the FARMacy Program must be current patients of Mon Health Preston Memorial Hospital who have a diagnosis of diabetes, have an HBA1C level of greater than 7, a BMI of greater than 30, agree to pre and post lab work, and commit to weekly participation in the program.
The program will be held every Friday from 10-11:30AM in at the hospital beginning June 28. The FARMacy Program is now accepting new enrollment through Friday, July 12.
