Diabetes Care

Mon Health Medical Center offers a National Diabetes Prevention Program that can help reduce your risk for type 2 diabetes.

Research has shown that making small changes – including making better food choices, increasing physical activity and reducing stress – can significantly reduce your risk for developing diabetes.

This program will educate and support you in making those important lifestyle changes. To participate in the program, you must be at high risk for developing diabetes, have been diagnosed by a physician as having prediabetes, or had gestational diabetes in a pregnancy.

Preregistration is required. It is not necessary to have a physician referral to participate as long as you meet the prediabetes requirements.
