Diabetes Education Series
The Diabetes Education Series at Mon Health Medical Center offers diabetes self-management education aimed at helping you manage your diabetes as well as possible. All of the classes are taught by a registered nurse (RN) and/or a registered dietitian (RD) holding the certified diabetes care & education specialist (CDCES) certification.
The courses in the Diabetes Education Series are:
Diabetes Self-Management Education (DSME) -- designed for people who are newly diagnosed with diabetes or who need to learn more about treatment options – including meal planning, physical activity, and medication. Patients are seen individually to assess learning needs and review medical history, and to create an education plan that best meets your needs. Sessions take place in a group or individual setting, depending on your needs, and consist of three to six sessions over three months with additional visits available as needed.
Insulin Education – designed for people with Type 1 or Type 2 diabetes who are on complex insulin regimens. The focus of this series of sessions is to coordinate insulin regimen with behaviors, such as eating and physical activity, to better manage diabetes.
Gestational Diabetes Education – designed for pregnant women who have been diagnosed with gestational diabetes. This two- to three-session course focuses on meal planning, blood glucose testing, and diabetes prevention following delivery.
If you are interested in participating in one or more of these classes, ask your healthcare provider for a referral.
