We understand, acknowledge and respect each individual's right to privacy. We recognize the importance of protecting the privacy of information provided by our patients, as well as general users of our website.
NOTE: Our Notice of Privacy Practices is a separate document regarding patient medical information, which is offered to all patients during the registration process.
Personal information privacy on our site
We do not ask for personal identification for any user of our site, nor do we collect it, without a site user's knowledge.
Forms & Surveys
We have many forms on our site that allow you to request information or action of us, or to give us feedback to improve our services and our website. Other than for online payment and registration purposes, to add you to our e-mailing list or to apply for employment or as a volunteer, your personal contact information will not be required. (Required fields are identified by a red star *.)
On other forms, we may provide fields for personal contact information, which you should fill in if you would like a personal response, such as to answer your question or request. These will be optional fields.
Personal information will be required for online Event Registration, online donations, payments and applications. Card and check information is not stored in our databases, but is passed to an accredited payment-processing service.
Rest assured that we do not provide any information supplied on our web forms to any outside organization for any reason, nor will we add you to our mailing list, unless that is specifically noted here or on the form.
In addition, we do not share any personally identifiable information with others, unless we must provide information for legal purposes or investigations, or if so directed by you - the user - through a proper authorization.
Your Responsibilities for Information Security
Though we take precautions to protect your information on our website, there are many ways that cyber criminals can gather your personal information through your Internet connection. You must use appropriate caution to protect your PC or mobile device and to use only secure wireless connections when you transmit confidential information. There are numerous respected websites that explain appropriate precautions that you need to take to ensure the security of your information and your devices.
You assume the risk for transmitting your personal identification, financial or other information over the Internet.
Email
We will not ask for personal information via email. The goal of a "phishing" email is to ask you for your personal information while disguised as a trusted source, such as a bank, a software vendor or maybe even as a hospital or other healthcare provider.
If you receive an email that looks like it is from Mon Health System or one of our affiliated services, which asks for your personal information, do not respond.
We will never request your password, user name, credit card information or other personal information through email.
Password-Restricted Online Access
At some point, you may sign up to use a limited-access area of our site. When you do, you take full responsibility to protect your userid and password to prevent its use by others.

The information provided on our site includes health content that may be useful to help you learn about or manage a particular illness or condition but is not meant to take the place of appropriate medical attention. Consult your healthcare provider whenever you have questions or concerns about your health.
